How to Provide this Gift
Send an e-mail using the contact form and please include the following information:
1. The name, phone number, and e-mail address of our new member (“the giftee”)
2. YOUR name, e-mail, and phone for invoicing and paperwork purposes
I will send you a custom welcome packet in the mail that you can print out and wrap however you’d like, and I will send an invoice to the e-mail you provide. The gift package will become active only when payment has been received.
If you don’t have time to wait for my e-mail
You may print out a generic form of the welcome packet by clicking here and wrap that up instead. Once you have e-mailed me all the informatoin and paid the invoice, I will send you a custom version of the packet in word that can be filled out and returned via e-mail.
If you want to purchase more than three months
You may purchase more than three months of the program if you like, but the entire gift package must be paid for up front. Simply let me know in your e-mail that you would like to purchase four, six, or whatever months and I will bill you accordingly. (the fee is $45 per month).
